Once the application has been approved, the applicant will be contacted directly by our distribution partner to discuss your requirements. There can be no alterations to the standard specification wheelchair except for sizing.
A small number of optional extras are available, these must be funded directly by the recipient of the wheelchair. Insurance of the wheelchair is the responsibility of the beneficiary and WheelPower accepts no responsibility for loss or damage caused to the wheelchair.
The Wheel Appeal will use the information you have provided in this application form to process your application for a sports wheelchair and training. Personal information in this form will be held for the purposes of applying for support from the Wheel Appeal. The only exceptions to this are: if you have given us permission to send you information about other activities or if we choose to include the information in your application in our publicity and campaigning work, printed materials, on our website, in the media or to provide information for current or potential fundraisers/donors that support our work.
By submitting an application, you agree that the Wheel Appeal may use any personal information relating to you and your family, including medical details and details of gender, age and ethnicity in the manner set out above.
By applying, you are also agreeing that the Wheel Appeal may store, disclose or transfer all your personal information contained in this application form to the employees of WheelPower and to any other person and organisation to the extent that it is reasonably necessary to proceed with your application or as otherwise required or permitted by law.
